Here is a list of some of the required documentation to become appointed with us. Send us an email so we can get you started on the appointment and onboarding process.

  • Oklahoma Health/Life Insurance License(s)
  • Copy of your Errors and Omissions Declaration Page
  • W9 Form
  • Copy of your AHIP Training Certificate (Medicare)
  • Copy of your FFM Training Certificate (Individual)

AHIP Certification

Complete your AHIP certificate with CommunityCare at a discounted rate! Agents who complete their AHIP certification through the CommunityCare link will no longer be required to send in a physical copy of their AHIP certificate.
