- Oklahoma Health/Life Insurance License(s)
- Copy of your Errors and Omissions Declaration Page
- W9 Form
- Copy of your AHIP Training Certificate (Medicare)
- Copy of your FFM Training Certificate (Individual)
AHIP Certification
Complete your AHIP certificate with CommunityCare at a discounted rate! Agents who complete their AHIP certification through the CommunityCare link will no longer be required to send in a physical copy of their AHIP certificate.
