Clinical Criteria
The CommunityCare Member Resource Center is a member-focused facility designed to enhance the quality of life for insurance plan members through personalized support services and a wide variety of activities. Open Monday through Friday from 8:00 AM to 4:30 PM, the center serves as a hub for plan assistance, health and wellness programs, social interaction, cultural engagement, and lifelong learning.
Support Member Services
Offer in-person plan education, enrollment support, and benefits assistance.
Promote Health and Wellness
Provide physical activity classes, screenings, nutrition workshops, and chronic disease prevention programs.
Enhance Social Engagement
Facilitate inclusive social events to foster community and reduce isolation.
Encourage Arts and Culture
Offer programs such as music, painting, crafts, and cultural celebrations.
Advance Lifelong Learning
Provide educational sessions on financial literacy, technology, safety, and aging well.
