To find participating physicians and facilities outside of Oklahoma:

CommunityCare Life and Health Insurance Company provides an in-network level of benefits for services delivered outside of Oklahoma through a national PPO network, PHCS. To find a participating provider outside of Oklahoma, follow the steps listed below.
