How do I choose a Primary Care Physician (PCP)?
When you enroll in CommunityCare HMO, you will
choose a PCP who will
manage and coordinate your healthcare needs. A list of our physicians is
available in a printed directory that you will receive when you enroll. You
may also visit the CommunityCare Web site at www.ccok.com to search for
physicians.
Your healthcare will be arranged within
your PCP’s network, which includes
specialists, OB/GYNs, hospital and mental health providers. You may change
your PCP within your network as needed.
Who do I contact if I need to change PCPs, request a new ID card, ask about my benefits or have other questions?
Please contact
Member Services at 594-5242, Monday-Friday, 8 a.m.-5 p.m.
A representative will assist you with PCP changes and selections, ID card
requests, benefit questions, address changes and more.
What if I have a question about my prescription drug benefit?
Please contact the
Pharmacy Help Desk at (877) 293-8628. Representatives
are available to answer your questions Monday-Friday, 8 a.m.-6 p.m.

pharmacy and formulary listings, printable benefit materials and more.
