The vision screening includes a brief history, vision and glaucoma screenings and a refraction for glasses. Most optometrists in
the vision network offer a 10 percent to 15 percent discount
for eyeglasses purchased at the optometrist’s
office.

(918) 594-5242 or 1-800-777-4890, Monday-Friday, 8 a.m.-5 p.m. or simply click on one
of the vision lists below. You do not need to contact CommunityCare or your PCP
before scheduling your routine vision screening. You will be charged your office copayment
for the screening. Remember to show your CommunityCare ID card at your office visit.
If your vision provider finds a potential medical problem with your eyes, you will
need to contact your PCP for further treatment options.
