CommunityCare HMO
CommunityCare HMO is an Oklahoma-owned and operated Health Maintenance
Organization (HMO) created by some of Oklahoma's premier
hospitals. CommunityCare HMO is owned and operated by
Saint Francis Hospital and St. John Medical Center in
Tulsa and provides a cost-effective, community-based
product. CommunityCare offers comprehensive medical
services through its own established network of local
hospitals, doctors and other healthcare providers.
CommunityCare Plus
CommunityCare Plus offers the basic elements of an HMO with the added benefit of allowing members to
self-refer outside the network. With its cutting-edge plan, the same managed care benefits are available while
still allowing members to self-refer for a reduced benefit.
Senior Health Plan
CommunityCare's Senior Health Plan is a program created especially for
Medicare beneficiaries. The U.S. government has contracted
with CommunityCare Senior Health Plan to provide Medicare
services. To get this contract, CommunityCare had to
demonstrate financial stability, solid healthcare experience
and an ability to choose and work with quality healthcare
providers.
CommunityCare65 Medicare Supplements
CommunityCare65 was designed to fill in the existing gaps found in current
Medicare coverage. Medicare Supplements are available
to individuals 65 and over, individuals under age 65
with certain disabilities and individuals with end-stage
renal disease. We’ve selected the three most
advantageous PlansPlan A, Plan C and Plan F.
CommunityCare Life & Health Insurance Company PPO Plan
CommunityCare Life and Health Insurance Company PPO Plan offers top-quality service and care
from a trusted local company. Every branch of its services -- from administration to claims to benefits -- is locally
owned. CommunityCare Life and Health Insurance Company PPO Plan offers a wide variety of plans that offer strong
benefits to members. It has a strong network of providers throughout Oklahoma and surrounding states, and its
arrangements with another PPO network for national coverage provides employers with the ability to cover employees
wherever they may be located.
Preferred CommunityChoice PPO
Preferred CommunityChoice PPO, Inc. is one of the managed care subsidiary companies of
CommunityCare Managed Healthcare Plans Of Oklahoma, Inc. Preferred CommunityChoice is a joint venture between
Saint Francis Hospital and St. John Medical Center in Tulsa. Preferred CommunityChoice provides managed care
to thousands of employees and their dependents through contracts with national insurance carriers, third party
administrators and direct contracts with self-insured employers.
Behavioral Health
CommunityCare mental health and chemical dependency services (outpatient
and crisis intervention) are available through self-referral
by calling 594-5262 in Tulsa or 1-800-774-2677 outside
the Tulsa area for authorization. PCPs may also refer
CommunityCare HMO members by calling CommunityCare Behavioral
Health Services at the above numbers. Behavioral Health
Services require precertification.
