The CommunityCare EAP and Behavioral Health Services Staff currently consists of the following individuals. Each has the specific experience, training, and credentials necessary to effectively perform their critical job functions.
Key Management Liaisons

Technical and Professional Qualifications (Tulsa Office)
Dr. Cage has been with the EAP since 1984. He has responsibilities on a variety of committees within CommunityCare and currently serves as chairman of the Benefits Interpretation Committee. His specialty is Stress Management Presentations and workshops. Dr. Cage is a licensed Clinical Psychologist and a Member of the American and Oklahoma Psychological Associations.
Terry joined the CommunityCare EAP staff in 1995 and now manages both the EAP and Managed Care departments. He is a State of Oklahoma Licensed Behavioral Practitioner (LBP) and, in addition to being a Certified Employee Assistance Professional (CEAP), has a Masters of Addiction Counseling (MAC). Terry's clinical experience in assessment and treatment qualify him as a Substance Abuse Professional (SAP) under federal government guidelines. He is also affiliated with the International Association of Trauma Counselors Association and is certified as a Disaster Mental Health Specialist by the American Red Cross. Terry has extensive experience with CISD’s throughout the state including assisting families in the recent Joplin, Missouri tornadoes.
Employee Assistance Program Staff
Linda joined the EAP staff in 2000. Her clinical experience includes individual, group and family therapy primarily in the area of substance abuse treatment. She has also worked in hospital settings as an Intake Counselor and in the private sector as an EAP Counselor. Linda has worked with Vietnam veterans and victims of abuse. Her areas of expertise are primarily in addictions and co-dependency counseling and counseling families of addicts/alcoholics.
Steve joined CommunityCare EAP in December 2010, having worked in the substance abuse and mental health fields for 23 years. He is a Licensed Professional Counselor (LPC), a Licensed Alcohol and Drug Counselor (LADC), & an Internationally Certified Advanced Alcohol and Drug Counselor (ICAADC). Steve specializes in working with substance abuse and co-occurring disorders. Most recently, he has been providing assessments for the Tulsa County Court system and training and supervision to counselors through the Oklahoma Department of Mental Health & Substance Abuse Services (ODMHSAS).
Elizabeth joined the EAP Counseling staff in 2010 and received her Master's of Science in Social Work degree from the University of Tennessee, Knoxville in May, 2001. Elizabeth is our bilingual counselor and speaks Spanish. Her clinical experiences include individual, group and family therapies. She has worked in a variety of levels of care including inpatient, outpatient, therapeutic foster care, group homes and school based. Areas of expertise include working with survivors of sexual abuse, trauma focused cognitive behavioral therapy, childhood behavioral issues and parenting skills.
LaKeia joined the EAP staff in 2011. She has a Master's Degree in Counseling Psychology from Northeastern State University and she is a Licensed Professional Counselor. In her professional career she has worked with children and families in therapeutic foster care, outpatient, and residential settings. She has expertise in working with domestic violence victims, perpetrators and children who have witnessed violence. Other areas of expertise include working with victims of trauma, domestic violence, trauma focused cognitive behavioral therapy, parent child interaction therapy, childhood behavioral issues, parenting skills and trauma informed care.
Jessica joined CommunityCare EAP in 2011. Jessica has extensive experience working with clients in a variety of settings such as residential treatment, public school systems and higher education, where she has served as a direct care provider, counselor and trainer. Her areas of expertise include substance abuse counseling and working with clients with anxiety and depression. Jessica holds a Master's degree in Clinical Psychology, and is both a Licensed Professional Counselor and National Certified Counselor.
Scott joined the Employee Assistance Program in 2007 and has 16 years of experience in the field of marketing. Scott serves as the direct contact for an EAP price quote. His primary responsibilities include all of the marketing coordination and EAP company account services, financial auditing and contract management. Scott’s main objective is to ensure the highest quality of customer service.
Amy joined the EAP staff in 2010. She has a B.S. in English and has over a decade of administrative experience. She is responsible for data collection, internal/external reporting, and billing; coordinating EAP Affiliate Provider contracts and maintaining records of licensure and insurance; producing the EAP quarterly newsletter; coordinating CommunityCare monthly in-house training classes. She also is the assistant to the BHS Senior Manager, provides back-up support for the front desk, and administrative support for the EAP staff.
Carol joined our staff in 2000. Carol has brought to our team a level of experience and professionalism vital in her contact with EAP companies and their employees. She is the first contact you will have when you call the EAP to talk to one of the staff members or when someone calls to schedule an appointment.
Managed Care Staff
Tricia joined the managed care staff in 1998. Her clinical experiences include individual, group and family therapy in a variety of levels of care. She has worked in hospital based treatment facilities (including inpatient, partial hospitalization and intensive outpatient programs), private practice, and community agencies providing outpatient services. Areas of experience include chemical dependency, depression and other mood disorders, women’s issues, spirituality, domestic violence with victims and perpetrators, survivors of sexual abuse, eating disorders, and facilitation of several parenting programs.
Judi joined the managed care staff in 2000 after transferring from the CommunityCare Outreach Department. Previously, Judi was employed as a social worker with Saint Anthony Hospital, City-County Health Department of OK County, Neighborhood Services Organization and Community Council of Central Oklahoma. Judi provides assistance to the EAP in the Oklahoma City area.
David joined the managed care staff in 2003. He has been in the behavioral health field since 1983 in a variety of settings including inpatient, partial hospitalization, intensive outpatient and outpatient. David has worked with both adolescents and adults in the community as well as in the private sector. David facilitates EAP workshops related to financial issues and mood disorders.
Jay joined the managed care staff on Nov. 1, 2010. His clinical experiences include individual, group and family therapy in a variety of levels of care. Jay has also worked in crisis stabilization for a community agency, for a psychiatric hospital providing acute level therapy & case management, and community agencies providing outpatient services. Jay has also worked in utilization management & review, evaluating clinical information to determine appropriate levels of care. Jay has also worked for the Oklahoma Forensic Center providing legal competency training.
Dawn joined the managed care staff in 1997. Dawn has held positions as the departmental assistant, Intake Specialist and Lead Intake. In 2001 Dawn became the Special Projects Coordinator. Her duties include a variety of tasks including reporting/data management, process improvement, interfacing with other departments within CommunityCare and assisting with implementation of system and process changes.
Kristal joined the managed care staff in 2001. Prior to CommunityCare she worked as a case manager, psychiatric technician, and direct care worker. Kristal is responsible for overseeing the coordination of all intake functions. She is the resource person for intake and clinical specialists in regard to policies and procedures. Kristal is also responsible for maintaining current benefit plan information.
Arlene joined the managed care staff in 2004. Her career in behavioral health started in 1998, working with a severely mentally ill population riddled with other factors (alcohol/drug addiction, homelessness, frequent incarceration). In 2001, she moved on to work at a residential rehabilitation facility treating traumatic brain injury patients with mental illness, where she provided direct care as well as recreational therapy. As a C.P.I. Instructor, she also taught Nonviolent Crisis Intervention to the staff. Arlene conducts intake, screening, and triage activities via telephone.
Dana joined the managed care staff in 2006. She has been a CommunityCare employee since 2000 as a Member Services representative, providing benefit information, assisting members in obtaining authorizations to specialists, answering billing inquiries, and other duties. Dana now conducts intake, screening, and triage activities via telephone.
