Vision Providers for CommunityCare Senior Health Plan Members


You may receive a vision screening exam once every twelve months. This includes a brief history, a visual field and glaucoma screening, and refraction for eyeglasses. You are also eligible to receive a standard pair of eyeglasses every 24 months. Our eyeglass benefit is limited to single vision lenses or standard lined bifocals with a standard plastic frame. If you want to upgrade the frames or lenses you simply pay your co-payment and the additional cost of the upgrade.

To obtain a list of routine vision providers in your area, simply use our online provider directory below, or call Member Services at (918) 594-5323 or 1-800-642-8065, Monday-Friday, 8 a.m. – 5 p.m. You do not need to contact CommunityCare or your PCP before scheduling your routine vision screening; authorization rules may apply. You will be charged your office co-payment for the screening. Remember to show your ID card at your office visit.

If your vision provider finds a potential medical problem with your eyes, you will need to contact your PCP for further treatment options.

To locate vision providers within the online directory, select "specialist" as the provider type and then select either "Ophthalmology" or "Optometry" in the specialty drop down menu.
