What is CommunityCare HMO?
CommunityCare HMO is an Oklahoma-owned and operated Health Maintenance Organization (HMO) created by some of Oklahoma's premier hospitals. We provide a cost-effective, community-based product.
CommunityCare offers comprehensive medical services through its own established network of local hospitals, doctors and other healthcare providers.
What Does CommunityCare HMO Mean for You?
- We are owned and operated by two premier hospitals in Oklahoma -- Saint Francis Hospital and St. John Medical Center in Tulsa.
- You can establish a relationship with just one doctor. This physician will coordinate all of your medical care.
- The HMO plan includes physician office visits with little to no copayments. There is no paperwork to file. All routine preventive care is covered, even benefits for hearing and vision screening.
- Comprehensive benefits include the 24-Hour Nurseline, a Health Information Service, Well Woman self referral and Vision Screening self referral. The 24-hour Nurseline
allows you to speak directly with a registered nurse about your health concerns, or you may choose to hear recorded information about health topics of your choice.

If you live or work in one of the counties listed below you are eligible to enroll or remain in the Commercial HMO plan.
The counties in the HMO service area are as follows: Adair, Canadian, Cherokee, Choctaw, Cleveland,
Craig, Creek, Delaware, Haskell, Latimer, LeFlore, Logan, Mayes, McIntosh, Muskogee, Noble, Nowata, Oklahoma,
Okmulgee, Osage, Ottawa, Pawnee, Payne, Pittsburg, Pottawatomie, Pushmataha, Rogers, Seminole, Sequoyah, Tulsa,
Wagoner and Washington.
