Below is a sample of the HMO Group Health Care Services Agreement (GSA) that is delivered to each new HMO client. This document is only a sample – a finalized GSA will be completed and delivered to the client after implementation of a new policy.
The GSA Delivery Receipt Form needs to be signed by the client and submitted with all other new group paperwork to CommunityCare.
