View the Summer 2009 Employer NewsletterCommunityCare is pleased to announce that CommunityCare Individual Health Plan’s rates, benefit information and application are now available at www.ccok.com. The Individual Health Plan is effective June 1, 2009.
Currently focused in the Tulsa area, Individual Health Plan offers a range of PPO plans that provide extensive benefits with office copays for visits to in-network primary care physicians and specialists, significant wellness and preventive care benefits, prescription drug benefits and care for other major services subject to the deductible level elected by the member.
The network for this product is based on our CommunityCare Direct PPO network, which has as its foundation the Saint Francis Health System and the St. John Health System. As is the case with all individual health plans, acceptance will be subject to underwriting evaluation and approval, and a pre-existing conditions provision will apply.
Our goal remains to provide accessible, affordable health insurance to as many Oklahomans as possible. We have worked diligently to make the premium rates competitively priced for these new plans. Each member of a family, even dependent children, will be allowed to apply for their own membership.
Appointed CommunityCare producers who desire to become involved with this new offering on either an individual basis or a list bill basis will have the ability to do so. Producers will be paid $75 per approved application. We will provide producers with a range of resources and contacts to minimize your time and involvement when assisting clients wishing to apply for our new individual health plans.
For more information about the Individual Health Plan, please contact Shirley Walker at (918) 594-5295, Ext. 6212.
