CommunityCare HMO is an Oklahoma-owned and operated Health Maintenance Organization (HMO) created by some
of Oklahoma's premier hospitals. CommunityCare HMO provides a cost-effective, community-based product.
CommunityCare offers comprehensive medical services through its own established network of local hospitals,
doctors and other health care providers.
CommunityCare Plus POS Plan
CommunityCare Plus offers the basic elements of an HMO with the added benefit of allowing members to
self-refer outside the network. With its cutting-edge plan, the same managed care benefits are
available while still allowing members to self-refer for a reduced benefit.
CommunityCare Life and Health Insurance Company PPO Plan
CommunityCare Life and Health Insurance Company PPO Plan offers top-quality service and care from a
trusted local company. Every branch of its services -- from administration to claims to benefits -- is
locally owned. CommunityCare Life and Health Insurance Company PPO Plan offers a wide variety of plans
that offer strong benefits to members. It has a strong network of providers throughout Oklahoma and
surrounding states, and its arrangements with another PPO network for national coverage provides
employers with the ability to cover employees wherever they may be located.
